By enrolling in or using the Remote Patient Monitoring (RPM) and/or Chronic Care Management (CCM) services provided by Community Strong LLC (“Community Strong,” “we,” “us,” or “our”), you (“Patient” or “you”) agree to be bound by these Terms and Conditions (“Terms”). If you do not agree with these Terms, you may not use our services.

2. Description of Services

Community Strong LLC provides RPM and CCM services as an adjunct to your existing healthcare.

  • Remote Patient Monitoring (RPM): This service involves the use of devices to collect and transmit physiological data (e.g., blood pressure, weight, blood glucose levels) to our care team. The data is reviewed to help manage your health conditions. This service is not a substitute for in-person medical care or emergency services.
  • Chronic Care Management (CCM): This service provides non-face-to-face care coordination for patients with two or more chronic conditions. This includes creating a comprehensive care plan, managing care transitions, medication reconciliation, and providing support between in-person visits.

3. Patient Consent

You hereby provide your informed consent to receive RPM and/or CCM services from Community Strong LLC. You understand that:

  • Your participation is voluntary, and you can withdraw from the program at any time by notifying us in writing.
  • The data collected through RPM devices will be shared with your healthcare providers for the purpose of your treatment and care management.
  • These services are not designed to address medical emergencies. In the event of a medical emergency, you must immediately call 911 or go to the nearest emergency room.

4. Privacy and Confidentiality

Community Strong LLC is committed to protecting the privacy and security of your Protected Health Information (PHI). We comply with all applicable federal and state laws, including the Health Insurance Portability and Accountability Act (HIPAA).

5. Patient Responsibilities

As a participant in our services, you agree to:

  • Use any provided RPM devices as instructed and ensure accurate and timely data transmission.

  • Promptly inform us of any changes to your contact information or health status.

6. Billing and Payment

  • Financial Responsibility: You are responsible for all applicable co-payments, deductibles, and co-insurance as determined by your health insurance provider.
  • Billing: We will bill your insurance provider directly for the services provided. You will receive a statement for any outstanding balance after your insurance has processed the claim.
  • Consent to Bill: By enrolling, you authorize Community Strong LLC to bill your health insurance provider on your behalf.

7. Disclaimer of Warranties

The services are provided “as is” and “as available.” Community Strong LLC makes no express or implied warranties regarding the services, including, but not limited to, the effectiveness of treatment or the outcomes of your health conditions.

8. Limitation of Liability

To the fullest extent permitted by law, Community Strong LLC and its employees, agents, and affiliates shall not be liable for any direct, indirect, incidental, special, or consequential damages arising from your use of the services, including any harm, injury, or loss of data. You acknowledge and agree that the services provided are for monitoring and management purposes and are not a substitute for professional medical judgment.
