We are excited to introduce our medical director Kathy Bella! With over 30 years of experience, Kathy Bella has a long and distinguished career in healthcare. As a Registered Nurse (RN) for 24 years, she spent 17 of those years in the Intensive Care Unit (ICU). For the past 9 years, Kathy has been a Nurse Practitioner (NP) in family practice, and she now serves as the Medical Director at Community Strong. She is dedicated to improving the lives of her patients and shaping the future of healthcare.

3. Print the referral form:
   Staff should print the completed referral form.
4. Fax the form to Community Strong;
   Our care them will review the referral.
   We’ll contact your patient within 48 hours.
